Name Change Checklist

Investigate your states policies on changing your name after you are married. Policies may vary from state to state. Typically, you cannot proceed without the Certificate of Marriage, which you can only apply for after your officiant and witnesses sign your Marriage License. Send off for that right away.

• Do first: Social Security     (SS5 Form Needed, Need Original or Certified Marriage Certificate)
• Do second: Driver’s license
• Do third: Bank accounts
